| Academic Progress Pre-Plan Survey | Student Forms | Undergraduate students on any type of academic probation or warning are requested to fill out the Academic Progress Pre-Plan Survey via their RamPortal account. | Legal Name Change Request Form | Student Forms | A student may change their name to match their ID/SSN card because of a data entry error or a change is necessary after a marriage or divorce. | Accelerated Bachelor’s to Master’s Degree Enrollment Request Form | Student Forms | This form is for undergraduate students (while working on their Bachelor's degree) to request a change of major to an accelerated program that leads to a graduate program. It is recommended that students discuss the completion of this form and their intent to pursue a graduate degree with their advisor before filling out this form. Students admitted to an accelerated program are provisionally accepted into the corresponding master's program and can continue directly into the graduate program following conferral of the undergraduate degree, conditional on satisfactory performance in prior courses.
